Dental and Vision Insurance is provided by Simple.  The enrollment/change application as well as Dental claim forms and Vision claim forms are found below.  Our Dental and Vision insurance covers dependents until the age of 19 unless they are a student and the coverage will continue until the age of 24.  Simple will not notify us when your dependent turns 24 - so please let me know so we can change your premium if                                                      need be.
